With this application you are requesting to stop utility service and close your utility account.

Service requests must be made a minimum of 24 hours in advance of date of termination.

Utilities may be turned off as early as 8:00 am on scheduled date of application.

Once off a $50.00 fee is required to restore service if additional service time is required.

Any deposit on account will applied as a credit against any charges due.
